Our Current Initiatives

Lead Screenings

What we do: Provide wrap-around support for lead testing and hazard follow-up so families aren’t left on their own after a positive screen.

Who we serve: Households with infants and children under 6, pregnant people, and caregivers in older housing or high-risk ZIP codes.

Services include:

In-home/clinic blood lead testing coordination and reminders

Environmental risk checks and referrals for home inspection/abatement

Navigation to pediatric care, WIC, nutrition education, and legal housing support

Supplies & education: cleaning kits, safe renovation tips, healthy home coaching

Follow-through: closed-loop tracking until hazards are addressed and follow-up tests completed

How it works:
Referral → Same-week contact → Test scheduled → Results explained → Home resources + partner handoffs → Follow-up testing until resolved

Chronic Disease Prevention

What we do: Connect adults and caregivers to screenings, risk reduction, and ongoing support to prevent or better manage chronic disease.

Focus areas: Hypertension, prediabetes/diabetes, high cholesterol, kidney disease, and stress/behavioral health.

Services include:

Screenings: BP, A1c, and risk assessments at clinics and community events

Benefits & access: Medicaid/VA enrollment, low-cost meds, transportation to visits

Navigation: Appointment scheduling, reminders, and closed-loop referrals to primary and behavioral care

Education & tools: Nutrition and movement coaching, quit-line linkage, self-monitoring tips

Social supports: Food and utility resources, caregiver respite, housing navigation

How it works:
Screen → Explain results & set a mini-plan → Warm handoffs (primary, behavioral, community) → 30/60/90-day follow-ups
